1,25-dihydroxy vitamin D (calcitriol) has been hydroxylated in the kidney and is the active form of vitamin D. This test should only be requested if 25-OH vitamin D results do not fit with the clinical picture. Measurement may be useful in the investigation of possible vitamin D-dependent rickets and in patients with hypercalcaemia to investigate possible excess 1,25(OH) vitamin D production e.g. granulomatous diseases (sarcoidosis, TB or lymphoma). Only routinely available to Endocrinology, otherwise please discuss with the Duty Biochemist (ext 26922 option 2).
