The feedback that is given by staff, carers and loved ones helps to improve the care given to patients at the end of life in hospital.
This is a national survey, and there is no obligation to take part if you do not wish to.
We would like to invite you to complete the survey if your loved one;
- died at the LGI or St James’s Hospital in 2024
- was over the age of 18 when they died
- did not die in the Emergency Department (A&E)
- did not die from a sudden event
This national survey is not looking at everyone who has died in hospital. Views are collected in other ways for suicide, maternal deaths and deaths of people with a diagnosis of learning disability.
