A booklet has been developed to support older people to be active and healthy at home.
Being active at a level that feels comfortable is unlikely to make your health worse; in fact it can help to manage many health conditions.
Support for carers
There are a number of ways in which the Trust supports carers as well as sharing links to other organisations and useful resources.
What we do
The Leeds Centre for Older People’s operates an ‘age’ and needs based policy – flexible from age 65 to 79 and comprehensive above 80. Patients are admitted via acute assessment wards at St James’s Hospital. Where early discharge is not possible patients are transferred to one of the inpatient wards as appropriate. There are approximately 12,000 admissions per year. The Leeds Centre for Older People works closely with Leeds Community Healthcare NHS Trust to improve and sustain integrated health and social care.
Inpatient Wards
Our inpatient wards include J7, J8, J15 and J17 and are located in Gledhow Wing, St James’s Hospital. The majority of our patients are admitted from the Emergency Department or via their GP. Once patients have been reviewed in our Acute Internal Medicine wards, they will be transferred to an Older People’s Medicine ward for further treatment. Our inpatient wards provide holistic nursing, therapy and medical support.
Discharge Wards
Wards J14 and J16 are designated discharge wards on Level 8 of Gledhow Wing on the St James hospital site of Leeds Teaching Hospitals Trust. These wards are designed to enable the specialist teams to continue planning discharge for patients who no longer require a hospital stay. Both wards provide nursing, therapy assessment and intervention as required with support from the medical team. J14 and J16 staff work together with patients and carers using a partnership approach to ensure that patients are able to self-manage and maintain a good quality of life.
Hospital Intermediate Care Unit (HICU)
HICU provides rehabilitation focused care to patients who are medically fit for discharge from hospital but not yet able to return to their usual place of residence as further rehabilitation is required. The unit provides an assessment and treatment service for individual’s requiring a level of medical and nursing intervention and/or rehabilitation. The unit is based in Beckett Wing, St James’s Hospital.
