Recognising the development of the Recovery Care Plan (RCP), a digital tool designed to support timely patient discharge and recovery at home. Part of HomeFirst, Leeds’ intermediate care transformation programme, the RCP supports communication within the multi-disciplinary team, streamlining discharge planning and improving the patient experience by centralising discussions and interventions in one shared digital space.
