The platform will help you manage your treatment plan and recovery, giving you easy access to all the information you need from your phone, tablet or computer.
How do I access the platform?
You’ll be invited to use the platform by a member of our team managing your care. They will send you further instructions by text, which will explain the simple steps you need to take to log in.
Read more about the text message you’ll receive, who this will be from and what it will say.
You’ll need your NHS log-in details to sign in, so if you don’t have a log-in yet, create one now on the NHS website. This will make sure you’re ready to go when you receive your invite from us.
What will I be able to see?
The IBD team will use the platform to share information with you. This will include:
- Contact information for the IBD helpline.
- Links to resources which provide more information about your medication.
- Links to questionnaires which help you prepare for your appointments and help us understand more about your symptoms if they flare up.
If you have any questions or concerns about your condition at any time, you’ll be able to contact the team by email or telephone.
The team will also use the platform to send you secure forms to complete. This will help you prepare for your appointments in advance from home. If your symptoms flare up, we’ll also use a secure form to help us understand more so we can decide on next steps.
We’re rolling out the new PHR gradually across our services and hospitals. In the future, you’ll be able to use more of the platform’s features to access information in your hospital health records and track your symptoms.
