Positive ANCA is associated with small vessel vasculitis. Indicated in Wegner’s Granulomatosis, Microscopic Polyarteritis, Churg-Strauss syndrome. Screening for ANCA-associated small vessel vasculitis with anti-MPO and PR3 AB. Positive results will have an ANCA test by indirect immunofluorescence.

Two patterns are identifed by indirect immunofluroescence (IIF) – cytoplasmic (C-ANCA) and perinuclear (P-ANCA). Levels of MPO/PR3 also correlate with disease activity. P-ANCA with anti-MPO are found in approximately 70% of patients with Churg-Strauss and Microscopic Polyarteritis. C-ANCA/anti-PR3 are found in approximately 85% of patients with Wegener’s.
