The Personal Health Record (PHR) patient portal, known as Portasana, will enable you as the patient and the clinical teams who care for you at the Leeds Teaching Hospitals NHS Trust to view and securely share information about your health, providing you with access to digital information leaflets, clinical advice and guidance without the need for a physical appointment.
Not all patients can gain access to the PHR patient portal, your health care provider will assess the level of care you need and will decide if you are eligible for this service.
Once set up, if you wish to share information about your health with the Trust, this will go directly into your Electronic Health Record known as PPM+ and will form part of your health record via the PHR patient portal.
Security of information
Confidentiality affects everyone. We as a Trust have a legal basis to gather, store and process large amounts of information on a daily basis. This includes health records, personal records and computerised information for the purposes of preventive or occupational medicine; medical diagnosis; or if the process is necessary for the performance of a task carried out in the public interest. This information is used by many people throughout the course of their daily work.
Our duty to protect information and confidentiality is taken very seriously. We are committed to taking all reasonable measures to ensure the confidentiality and the security of all information for which we are responsible, whether computerised or on paper. This includes regular staff training on the legal obligations they have to maintain confidentiality and security of information at all times
What personal information is required for the portal registration?
In order to use the PHR patient portal, you will be required to register for the NHS Login, providing your consent. To sign up to the NHS Login, you will be required to provide some personal details about yourself, such as:
- Your name
- Your NHS number
- Your date of birth
- The Postcode of where you live
How your personal information is used
The clinical advice and guidance you may receive and any information you share about your health via the PHR patient portal will form part of your health record at the Leeds Teaching Hospitals NHS Trust. Your records at the Leeds Teaching Hospitals NHS Trust are used to manage and deliver the care you receive to make sure that:
- The doctors, nurses and other healthcare members of staff involved in your care have correct and up to date information, to look at your health and decide on the right care for you
- Healthcare staff have the information they need to be able to look at and improve the quality and type of care you receive
- Your concerns and worries can be properly investigated if a complaint is raised
- The right information is available if you see another doctor, or are referred to a specialist or another part of the NHS.
Who do we share personal information with?
Everyone working within the NHS has a legal duty to keep information confidential. Similarly, anyone who receives information from us has a legal duty concerning your confidentiality. On occasion we may need to share your health record to help care for you outside of the Leeds Teaching Hospitals NHS Trust, such as your GP or other health care providers.
While the Trust will require the use of Portasana as the data processor to provide you with this service, Wellola who provides Portasana are unable to see and/or amend your health record.

Opting out of the PHR patient portal
You as the patient have the right to Opt-out of the PHR patient portal at any time by deleting your PHR Account via the PHR app
SMS text and Email messaging
Should you be eligible to access the PHR patient portal, you will be required to register. To register, your health care provider will request an email address and/or a phone number that accepts text messages from you in order to provide you with a link. The link will then take you to the NHS app.
Proxy Access
Once signed up to the PHR patient portal, Proxy access allows you to give temporary access to your record to other people that you may wish to act on your behalf. You as the patient will have control over what level of access you allow. You can revoke this access at any time together with the level of access via the PHR patient portal.
